Is an Integrated Shield Plan worth paying for on top of MediShield Life?

Nurse here, seen both sides of the ward. My honest take: MediShield Life alone covers you well for B2/C class in public hospitals, which in Singapore is genuinely good care, you will NOT be turned away or get bad treatment in a public ward. The IP buys you: private hospital or A/B1 class public (own room, choice of specialist, shorter waits for non-urgent stuff). Is it worth it? Depends on what you value. My framework: 1) The base IP (public hospital A/B1) is reasonably priced when young and worth having for the higher claim limits and ward choice. 2) The RIDER (that covers your deductible/co-insurance so you pay near-zero) is where premiums balloon as you age, and after the reforms you now co-pay a portion anyway. 3) Private hospital IPs are the priciest and premiums get brutal past 60. My practical advice: get at least a public-hospital-tier IP while young and healthy (pre-existing conditions get excluded later, so lock it in early), use Medisave to pay the premiums where allowed. Skip the fanciest private-tier plan unless you strongly want private hospitals and can afford rising premiums for life. Don't let an agent upsell you the max, match the plan to whether you'll actually use private care.
